THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
Aesthetic Lab is required by law to maintain the privacy and security of your protected health information (“PHI”), to provide you with this Notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We will follow the terms of this Notice currently in effect.
We reserve the right to change this Notice at any time. Any revised Notice will apply to all PHI we maintain and will be posted on our website with an updated effective date.
How We Use and Share Your Information
We may use and disclose your PHI without your written authorization for the following purposes:
Treatment
To provide and coordinate your care.
Example: Sharing information with another provider involved in your treatment.
Payment
To bill and receive payment for services.
Example: Submitting claims to a third-party payer if applicable.
Healthcare Operations
To run our business and improve care.
Example: Reviewing treatment outcomes, training staff, and managing operations.
Other Uses Allowed by Law
We may also use or disclose your information when required or permitted by law, including:
- Public health and safety activities
- Health oversight activities
- Law enforcement purposes
- Judicial and administrative proceedings
- Serious threats to health or safety
- Workers’ compensation
We limit uses and disclosures to the minimum necessary.
Uses Requiring Your Permission
Other uses and disclosures of your information, including most marketing uses, will be made only with your written authorization. You may revoke your authorization at any time in writing.
How We Contact You
We may contact you by phone, text message, or email for appointment reminders, treatment follow-ups, and service-related communications. You may request alternative methods of communication at any time.
Special Protections for Substance Use Disorder Records
To the extent that we receive or maintain substance use disorder patient records that are protected under 42 C.F.R. Part 2, we will comply with applicable federal confidentiality requirements.
Such records may be subject to stricter protections than other medical information. To the extent that we have your substance use disorder patient records subject to 42 C.F.R. Part 2:
- We will not use or disclose such information for treatment, payment, or healthcare operations without your written consent, unless otherwise permitted by law.
- We will not share that information for investigations or legal proceedings against you without your written consent or a court order and subpoena, as required by law.
Where applicable law provides greater privacy protections, we will follow the more stringent requirements.
Redisclosure
Information disclosed by us may be subject to redisclosure by the recipient and may no longer be protected under federal privacy laws.
Fundraising Communications
If we use your information for fundraising purposes, you will be provided with a clear opportunity to opt out of receiving such communications.
Your Rights
You have the right to:
- Access and obtain a copy of your medical records
- Request corrections to your records
- Request a list of certain disclosures of your information
- Request restrictions on how your information is used or disclosed
- Request confidential communications
- Receive a paper copy of this Notice
Your authorized personal representative may exercise these rights on your behalf as permitted by law.
Changes to This Notice
We reserve the right to update this Notice at any time. Updates will be posted on our website with a revised effective date.
